    Understand

    Plain-language explanation of what hospice is, what it is not, and what choices remain with the patient and family at every point.

    Evaluate

    Clinical eligibility is reviewed against recognized hospice criteria, with the attending physician and, when authorized, the hospice medical director.

    Coordinate

    Documentation, medication and equipment logistics, facility communication, and level-of-care decisions are organized in one accountable pathway.

    Support

    Ongoing symptom management, caregiver teaching, psychosocial and spiritual support, and bereavement services for the family.

Levels of care

The four hospice levels of care, described accurately

Under the hospice model of care, four levels are defined nationally. The level used at any time depends on documented clinical need, the plan of care, and available authorized resources.

Routine Home Care

The most common level of hospice care. An interdisciplinary team makes scheduled visits wherever the patient calls home — a private residence, assisted living community, or nursing facility.

Visit type and frequency are set by the plan of care and adjusted as needs change. Routine home care does not include continuous bedside staffing.

Continuous Home Care

A time-limited level of care intended for brief periods of medical crisis, when predominantly nursing care is required for several hours in a day to manage acute symptoms at home.

Eligibility is determined by documented clinical criteria and reassessed frequently. It is provided only for as long as the crisis criteria are met.

Inpatient Respite Care

Short-term care provided in a contracted facility so that a family caregiver can rest or attend to other obligations.

Respite stays are limited in duration under hospice benefit rules and depend on available contracted beds at the time of request.

General Inpatient Care

A short-term level of care for pain or symptoms that cannot reasonably be managed in other settings, delivered in a contracted hospital or inpatient unit.

Admission requires documented clinical need and is reassessed continuously, with the goal of returning the patient to their preferred setting when symptoms are controlled.

Availability and eligibility

Service availability, eligibility, coverage, payment, and admission depend on current authorization, clinical review, patient choice, and capacity.

What around-the-clock access means

A structured on-call pathway so calls at any hour reach a qualified person who can triage the concern, document it, and escalate to the appropriate clinician under the plan of care.

What it does not mean

It does not mean continuous bedside staffing. Continuous nursing care is a distinct, time-limited level of care with documented clinical criteria, and no specific response interval or admission outcome is guaranteed.
